Your USDOT number and legal name exactly as they appear on your MCS-150 (biennial update).

Your fleet size—the number of power units you operated in interstate commerce. This determines your fee bracket ($46 for 0–2 units up to $44,836 for 1,001+).

Your base state (principal place of business). Non-participating states (e.g. Florida, Arizona) must file through a participating state.
